A patient’s health insurance is a contract between the patient and their insurance company. As such the patient is responsible for understanding the policy reimbursement requirements. If your health insurance is with a company with which we participate and you have supplied insurance information, we will bill your insurance carrier for you. Patient co-payments are due at the time of your visit.
Most Health Maintenance Organizations (HMO) and AHCCCS plans require a patient to have a referral form from their primary care physician prior to receiving specialist treatment. HMO and AHCCCS patients must ensure that the referral is received by our office prior to the visit.
